REGISTRATION
INFORMATION
The following information and completed forms will be
required of all incoming students prior to becoming students of
1. Student Family Information
2. Verification of residence.
3. Health History.
4. Emergency Medical Authorization.
5. Internet Permission Form.
In addition parents are requested to furnish;
1. An original birth certificate (original will be
returned).
2. Where appropriate, documentation that establishes
the residence of the parent registering the student as the legal primary
custody holder of that child.
3. Proof of immunizations.
